Community Health Services

Community Health provides a range of services for all age groups.

Contact Community Care Coordination Team to determine your eligibility to access these services and to be connected with the therapy, information or service that is right for you.

Our services can also be accessed through the My Aged Care referral and assessment process or restricted by other relevant funding guidelines. However MHS can always be contacted for urgent assistance.

Telehealth options are available.

Community Health fees vary according to the Commonwealth or State program or service you are accessing. Concession or eligibility for a program of care determines whether fees will be charged and at what level. See the Community Health Schedule here
