District Nursing
Moyne Health Services’ Registered Nurses deliver District Nursing Services to people in the Koroit and Port Fairy region. Health care and support to clients is provided in their own home or at the Community Health Centre.
The professional and experienced staff work in partnership with local GPs and other health providers to ensure care is well planned and coordinated.
Services delivered by District Nursing may include but are not limited to:
- injections
- wound management
- observations and monitoring
- blood collection
- clinical assessment
- bowel and bladder management including stomal, continence and catheter care
- health education for clients and families
- supportive health care
- palliative care in the home
- referral to other services.
Service hours
District Nursing Services are available Monday to Friday from 7.30am-3.30pm.
Weekend and after hours service can be provided in special circumstances.
Eligibility
The District Nursing Service is available to all people. However, access is determined by program funding and current demand on the service.
All referrals are prioritised according to need. People who are at-risk are likely to be prioritised to access services.
Access
Referral to the District Nursing Service is via a hospital admission, community support services, carers, GPs or self-referral.
